If you lose your Covid Vaccination Card -How to get a replacement copy in Florida

There are some situations where you are required to show proof of vaccinations. Don't lose your card...but if you do, this is how you can get a copy in Florida.

1. Ask your vaccine provider - all providers that administered vaccinations, including pharmacies, have access to FLORIDA SHOTS, a statewide central system that keeps records.

2. Contact your county health department - FL Dept of Health Miami-Dade 786 845-0550 and FL Dept of Health Broward 954 412-7300 and FL Dept of Health Monroe 305 293-7500.

3. If you are at least 18 you can submit your request directly to FLORIDA SHOTS.
