1. ACCEPT SUPPORT – Our Comforcare Certified Dementia Practitioner (CDP) will assess/evaluate and implement a customized plan of care. A CDP is someone with a certification demonstrating achievement and specialized training in the areas of Alzheimer’s and dementia care. The CDP can offer patients/families coordination of health care services, information, support and resources.
2. DEMENTIAWISE SPECIALIZED TRAINING – Our Comforcare Alzheimer’s and Dementia care program offers specialized person-centered care designed to implement current proven practices and strategies for dementia care to create a supportive and therapeutic environment. DementiaWise is recognized by the Alzheimer’s Association for incorporating the evidence-based Dementia Care Practice.
3. STIMULATE REMAINING ACTIVITIES – Our DementiaWise trained caregivers provide ongoing personal care and companionship, encouragement, support and meaningful activities that stimulate your loved one’s remaining activities.
